To receive a chapter stipend for the 2009-2010 academic year, the following items need to be submitted to
Maggie Motley by October 15, 2009.
- Update U.S./International Chapter Checklist
- Approved/Signed U.S. OR
International
Operating and Procedural Guidelines Please complete and sign these forms.
- Annual Financial Report. Please record your chapter's financial activity
through June 30, 2009. This can be in the form of a bank statement, Excel file, Word document, or other
applicable format.
- Financial
Information Update. Please complete and sign this form. NOTE: A copy of your
chapter's bank statement must be included.
- Approved/Signed Group Exemption Authorization Form. Please complete and sign
this form.
- Chapter Roster. Please include the names, titles, email addresses, phone
numbers and mailing addresses of your chapter advisor and student executive committee.
One main mailing address for the chapter is also required. Additionally, we need
a list of all chapter members (AAPS and non-AAPS members), including their email
and mailing addresses. Please note that at least 10 of your chapter members must
be active AAPS members, including chapter leadership. Also, your faculty advisor
must be an AAPS member.
- Proposed Activities/Events List. Please include a list of your chapter's
planned activities and events for the upcoming school year, including corresponding
dates if known (general timeframe is fine).
- Updates for AAPS website. In April 2004, AAPS developed individual web pages
for each student chapter in an effort to provide them with a stronger voice and
presence on the AAPS Pharmaceutica site: (http://www.aapspharmaceutica.com/StudentCenter/studentchapters/studentchapters.asp).
We will be reviewing these web pages annually and would like you to provide us with
updates to the following information:
- Chapter Website: If your chapter has a website aside from the new AAPS page,
and it is not currently posted, please provide us with an updated link.
- General Chapter Information: We are looking for a paragraph of text that
provides a brief history or overview of your chapter. You can discuss when it was
initiated, chapter mission/values, overall goals, etc. As this section will not
change from year to year, please refrain from using any time-specific information.
- Chapter Activities: Please provide us with a list of activities (dates included)
of the major events/seminars/meetings/etc. that your chapter will be hosting or
participating in. Please include a list for the 2008-2009 school year AND the 2009-2010 year.
- Chapter Picture: If your chapter has a recent photo in .jpg format, please
include it with your information.
